Simultaneous Reconstruction of a Subtotal Maxillectomy and Columella Deficit using Radial Forearm and Preauricular Free Flaps

Reconstruction of subtotal maxillectomy defects with columella deficit is challenging. We report a unique case facial reconstruction using free radial forearm flap and preauricular for the deficit. A 73-year-old woman was diagnosed recurrent sebaceous carcinoma nose. performed wide excision, including areas right cheek, nose, upper lip, maxilla, columella. The resultant defect reconstructed three-dimensional flap. First, transfered to resurface nasal, oral, external side. Second, positioned into anastomosed distal portion pedicle. two flaps survived, patient recovered uneventfully. believe double pedicle connection method were effective in reconstructing present defect.
